How to ship cases to us |
| 1. Please prepare the following items before sending the case or a
digital case: |

a. Write an ordersheet to us, fill it with as much detailed
information as possible; such as dentist name, patient name, patient age, return date, tooth
number, service items needed, single crowns or bridge, color, etc. |
| b. Working impression (model) and opposite impression (model),
study impression (model) if necessary |
| c. Bite registration (if necessary) |
| d. Please include all implant parts if the case is an implant,
including lab parts |
e. Prepare airway bill for shipping (you can choose from the
following shipping couriers, such as DHL, UPS or Fedex), You may
also need to prepare a commercial invoice and a non wooden
declaration paper. Please contact our service people, we have those
format if needed. |
